We want to know what you think about Dementia Oxfordshire!

Do you have a few minutes to tell us about how you use Dementia Oxfordshire services? Are you happy to let us know what you think about the services we provide?

We are asking everyone who has had any contact with Dementia Oxfordshire to fill in a short survey. We would like to hear from people who are living with dementia and the people who help care for them.

It should take no more than ten minutes.

The survey is completely anonymous, unless you choose to give us your name and contact details so we can contact you for further comments.

Even if you do give us your details, completing the survey will in no way affect how we support you.

Why do we want to know what you think?

Our aim is to support people across Oxfordshire to live well with dementia. We also want to support those who care for people living with dementia.

The survey will help us understand which of our services people use, and which ones they find most helpful.
