Who can be referred?
Dementia Oxfordshire supports all people with a diagnosis of dementia who do not live in residential care and are either living in Oxfordshire or are registered at an Oxfordshire GP.
We accept referrals from the health care sector and voluntary sector, as well as self-referrals from people with a diagnosis, their family and carers.
How to make a referral
